Legal basis and things to bear in mind

The rectification of registered gender is highly protected data (Art. 83 of Ley 20/2011 of the Registro Civil) and will not be publicised without your authorisation.
You may also request a full transfer of your registry page, so that your record starts on a new one. It is not automatic: it is requested at the first appearance and is only available if your birth registration is governed by the 1957 Ley del Registro Civil (Civil Registry Act). We raise it with you when preparing the case file.
Standard literal certificates can be issued from the new record without any mention of the change appearing, unless expressly requested or under special authorisation.
If you live outside Spain, the case file is handled by the Oficina Consular (consular office) of the Registro Civil for your district, and both appearances take place there. You may choose an Oficina General (general office) in Spain instead, but then both appearances take place in Spain.
No certificado de empadronamiento (municipal census registration certificate) is required: the Instrucción of the DGSJFP of 26 May 2023 rules it out, because the application may be lodged at any office of the Registro Civil.
This procedure changes your gender and name in the Spanish Registro Civil (Civil Registry) and, with it, your DNI (which keeps the same number), your passport, and any degrees or certificates you ask to have reissued. If you live in another country, it does not by itself grant legal recognition there: each country has its own route and it is handled separately.
